Fellowship Assessment Forms

The ACGME has called on medical subspecialty organizations to develop consensus assessment instruments for use in fellowship training. As a response to the ACGME, and from requests from program directors to have forms that work easily with the Milestones,1 the ACLP Fellowship Education Subcommittee has designed three consensus assessment forms:

The Guide to the Assessment Forms describes the measurements of, and includes, each of the three forms, which can be also be downloaded separately with the links above.  The Guide and the forms are all MS Word documents.

You are not required to use these forms, but rather encouraged to use them. They are short, concise, and easily printed or copied into New Innovations or your hospital evaluation system. The assessments are available on both MedHub and New Innovations.

Form for the assessment of faculty

The ACGME specifies the domains which are to be evaluated annually for faculty in fellowship programs. These domains were changed in the Program Requirements which took effect in July 2019:

V.B.1.a) This evaluation must include a review of the faculty member’s clinical teaching abilities, engagement with the educational program, participation in faculty development related to their skills as an educator, clinical performance, professionalism, and scholarly activities. (Core)

V.B.1.b) This evaluation must include written, confidential evaluations by the fellows. (Core)

The Fellowship Education Subcommittee has developed a Faculty Evaluation Form which incorporates all of these domains and is available for the use of Program Directors or their designates. This form might be used also to assess new faculty to be added to the program, meeting the requirements for the Program Director to:

II.A.4.a).(4) develop and oversee a process to evaluate candidatesprior to approval as program faculty members forparticipation in the fellowship program education and at least annually thereafter, as outlined in V.B.;
